Medication Management

With the added complexity brought about by challenges in the economic environment, we have engaged the services of a team of medicines professionals.

We have a Clinical Pharmacists that are on-site for a number of days during the week. She has primary responsibility for medication reviews for all patients, and will periodically contact patients about adjustments to some of their prescribed medications. In addition to this, she manages the medications for the 2 care homes in the PCN.

The Prescription Clerk role has originated to assist the both the clinical team at the practice and the wider community with sourcing appropriate medication as prescribed. She works with the pharmacist to coordinate with patients and pharmacies the appropriate use of medication.
